Job Description & Requirements Title: Occupational Therapist (OT) – Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM – 5:00 PM)
Seeking an experienced Occupational Therapist for Home Health/Hospice care. The role involves managing a caseload of geriatric patients within a defined territory and providing high-quality, timely therapy services. Candidates must be comfortable working independently, traveling within the assigned area, and maintaining accurate documentation.
Job ResponsibilitiesProvide occupational therapy services to geriatric patients in a home health/hospice setting
\tManage an average caseload of 5–7 patients per day
\tComplete Start of Care (SOC) documentation when required
\tEnsure timely, accurate documentation and clear communication
\tCollaborate with interdisciplinary care teams
\tParticipate in on-call rotation (approximately every 6th weekend)
\tTravel within a territory up to a 60-mile radius
\tComply with dress code and professional standards
\tMinimum 1 year of recent OT experience
\tHome Health and/or Hospice experience required
\tOASIS experience required for SOC visits
\tProficiency with HCHB (Homecare Homebase) EMR required
\tBLS certification required
\tActive OT license in hand at submission
\tValid driver's license and reliable personal transportation
\tAbility to be flexible with territory assignments
\tStrong documentation and communication skills
\tNO first-time travelers
